Immunization Requirements
State law requires children to be immunized against certain vaccine-preventable diseases to attend school in Texas. Getting your child vaccinated protects your child's health and that of the community.
Students cannot attend school without the appropriate documentation for the required vaccines or a valid exemption. (Texas Administrative Code §97.63)

Immunization Exemptions
Medical:
Physicians may write a statement indicating a vaccine would be medically harmful. The statement must be submitted to the school nurse. Unless a lifelong condition is indicated, the exemption is valid for one year.
Reasons of Conscience (including religious beliefs):
Parents may obtain affidavit forms from the DSHS website. Forms must be notarized, and the school must keep the original on file. The affidavit is valid for two years. In the event of a disease outbreak, an unimmunized child may be excluded from school.
Because the student had the disease:
Parents may submit a physician note indicating the date the child had chickenpox (varicella), or the physician may document it on the immunization record.
