HPISD Loves Its Volunteers!

Parent volunteers are an invaluable and essential part of the success and efficient operation of the Highland Park Independent School District. The amount of parental involvement in Highland Park ISD is what makes our schools unique.

To volunteer, each parent must complete this form prior to volunteering at any district activity. (ie: Carnival, Food Service, Field Trips) Click HERE to access the form.

VOLUNTEER REGISTRATION AND CONSENT FORM

District policy requires annual criminal history checks for all parent and community volunteers who work in our schools. This includes, but is not limited to, volunteers who work in the cafeteria, the school bookrooms, supply rooms, library media centers, supervise field trips, and teacher workrooms. We appreciate your assistance in this matter and for helping us ensure that all children are educated in a safe and secure environment.

CONSENT FOR CRIMINAL HISTORY CHECK

The criminal history record, as received from the reporting agencies, may include arrest and conviction data. I understand that this information will be used, in part, to determine my eligibility for a volunteer position with the district. I also understand that I will have an opportunity to review the criminal history, and a procedure is available for clarification if I dispute the record as received.

I/We, named below, do, for myself/ourselves, my/our heirs, executors, and administrators, hereby release and forever discharge and agree to indemnify and hold the HPISD and the reporting agencies, and each of their officers, directors, employees, and agents harmless from and against any and all causes of actions, suits, liabilities, costs, debts, and sums of money, claims and demands whatsoever, and any and all related attorneys' fees, court costs, and other expenses resulting from the investigation of my/our background in connection with my/our application to become a volunteer.

By providing the information below, I hereby give my permission for the Highland Park Independent School District to obtain information relating to my criminal history record through the Department of Public Safety and other agencies. I understand the information I am providing about age, sex, and ethnicity will not be used to determine eligibility for volunteerism but will be used solely for the purpose of obtaining criminal history record information. This is required by the Department of Public Safety and other agencies to ensure the accuracy of the report.
