SB 12 – Parental Consent for Services
In accordance with recently passed Texas Senate Bill 12, Highland Park ISD is newly required to provide this annual notice of the routine, campus-based physical and mental health-related services available to students during the upcoming school year.
These services are not new services and are provided by qualified school staff, including nurses, athletic trainers, and counselors. As a parent/guardian, you have the right to withhold consent (opt-out) or to grant consent (opt-in) for all health-related services provided at your child’s campus.
How to Complete the Consent Form in Skyward
- Log in to your Skyward Family Access account
- Find the Online Form labeled ‘25-26 HPISD Notification & Consent for Services’
- Indicate whether or not you consent for each child
